Checklist for Scholarship Success

I have:

Filed my Free Application for Federal Student Aid (FAFSA) form online at www.fafsa.ed.gov
Noted the Early Bird and Final deadlines for submitting my OSAC application and where to send updates to my address and school choice if needed.
Reviewed the 2009-10 Oregon Scholarship eApp Workbook or the OSAC Scholarship Catalog of programs:
  • Section One — Scholarships listed for graduates of Oregon high schools, regardless of the year of graduation. Scholarships are open to graduates of all Oregon high schools, multiple counties, and single counties.
    Section Two — Scholarships listed by academic interest, career choice, and for specific populations (foster youth, student parents, part-time, GED and home-schooled students, etc.)
    Section Three — Scholarships sponsored by employers or membership organizations
    Section Four — List of scholarships by donor or honor name.
Listed on the eApp worksheet the scholarships to follow-up on that had more requirements online.
Completed the worksheet before entering information in my eApp.
Completed the electronic application (eApp) online. (A paper application can be ordered by calling OSAC at 800-452-8807.)
Completed the required Activities Chart as part of the eApp. Completed the required Activities Chart as part of the eApp. instructions in the Scholarship Workbook, page 36, item #14, or online.
Completed my 150-word essay answers to the four required short essay questions.
Double-checked that I have provided the correct transcript(s) by scanning and uploading to my eApp (preferred), or by having my school transmit directly to OSAC. (See transcript transmittal and scanning options)
Paper applicants, only: Attached additional documents required by specific scholarships.
Paper applicants, only: Signed, dated, and assembled my application packet.
Printed or saved a copy of my complete application packet.
Blackened the first five digits of my Social Security Number on all packet documents, except for the application form.
Submitted my eApp and required documents online or mailed/delivered my paper application packet in advance to ensure it arrives at OSAC by the FINAL March 2, 2009 deadline or mailed/delivered by Monday, February 16, 2009, to be entered in the Early Bird review and drawing for a $500 scholarship.

The three main reasons OSAC applications are rejected:

  1. Missing or incomplete transcripts
  2. Missing signature on the paper application
  3. Late application packet

 

Your eApp or paper application and required documents must arrive at OSAC by the appropriate deadline:

Monday, February 16:  EARLY BIRD Review

Monday, March 2:  FINAL Deadline. No Exceptions

If you wish to be considered for the Early Bird scholarships and an early review of your application, your deadline for submitting your complete application packet is Monday, February 16.
If you need to know that OSAC received your scholarship materials, use a delivery service that gives you a return receipt. Because of the volume of materials that OSAC receives, we are unable to provide you with this information.
