Special Requirements

Some scholarships listed in the 2009-10 Oregon Scholarship eApp Workbook require you to provide extra documentation.  In the Workbook, each scholarship features a tool bar, which may contain the term “XDoc” to indicate additional documents are required. This may be an additional essay, names of references, proof of relationship, proof of U.S. birth, or other documentation a donor requires—e.g., the Ford Special Recommendation or ReStart Reference forms.

You must review the scholarship eligibility criteria online at www.GetCollegeFunds.org/eapp.html to determine what type of document must be submitted with your application.

If you are submitting the eApp, you are encouraged to scan the document (e.g., birth certificate, proof of veteran status) and upload to your eApp at the prompt.  If you can't upload the document, specify the reason why you can't upload, print the eApp-generated cover sheet, and mail/deliver to OSAC.

If you are completing the paper application, submit the document with your application packet, but make sure to write your name, the last four digits of your SSN, and the document description on the top of each page.

If you are applying for programs (Crawford and Steinbach) that both require proof of U.S. birth and you are applying using a paper application rather than online, send only one copy of your birth certificate or passport.
