OSAC Templates for the 2008-09 Scholarship Application

Activities Chart and Four Required Short Essay Answers

The OSAC 2008-09 Scholarship Application requires that all applicants attach their Activities Chart, four Short Essay Answers, and transcripts to their signed scholarship application before sending to OSAC.

To create your Activities Chart and four required Short Essay Answers, you may use the forms in the online application (eApp), you may use the templates provided below, or you may create your own. However, if you create your own, you must follow the correct formats (see Activities Chart and Essays for more on formatting).

For your convenience, the templates are available in several formats:

PDF (or .pdf – portable document format) – Use this form to fill in your activities or essay answers and then print the form (you can’t adjust the columns or save your information).  To use this, you will need Adobe Acrobat Reader on your computer.  If you do not have it, download it for free by clicking on the link below and following the instructions.

Word Doc (or .doc – Microsoft Word document)– Use this if you can open Microsoft Word documents and you want to either save the document on your computer or would like a little more flexibility (e.g., want more space for your Volunteer activities and less space for your Work activities).

RTF (or .rtf – rich text format) – Use this if you cannot open Microsoft Word documents and can’t or prefer not to use PDFs.
