Templates for the 2010-11 Scholarship Application

Activities Chart and Four Required Personal Statements

The OSAC 2010-11 Scholarship Application requires that all applicants submit their Activities Chart, required Personal Statements (150 words or 1,000 characters), transcripts, and other required information by the appropriate deadline.

For eApp users: You will complete your Activities Chart and four Required Personal Statements as part of the application. You can also upload your transcript(s) and other required documents as .pdf files (.jpg, .tif, and .gif also accepted) to your eApp when prompted.

eApp users can:

  1. Create the Activities Chart and four required Personal Statements by typing information directly into the eApp online application.
     
  2. Download templates provided below and copy/paste text into your eApp. The essay answers must not exceed the 150-word (1,000 characters) limit, otherwise, it will be truncated and text will be cut.
     
  3. Save their CIS / OSAC Activities Chart in a word processing program and then copy and paste information into their eApp Activities Chart. EApp users can also enter the information directly into the eApp Activities Chart. To ensure OSAC receives a complete application, eApp users are strongly discouraged from printing their Activities Chart and mailing it to OSAC as a separate document.

Paper applicants only: You may also create your own Activities Chart or download the template below. If you create your own, follow the correct format as shown in the template or example below, or in the Scholarship Workbook, page 36. For your convenience, the templates are available in several formats, below.

If you create your own Activities Chart, follow this layout format.

Enter each activity under the categories:

  1. School Activities
     
  2. Volunteer Activities (Community/Family)
     
  3. Paid Work History

You must include:

  • Activity Title (e.g., Chamber choir, Volleyball, Red Cross blood drive, Pizza Hut)
  • Dates you participated (from—to)
  • Time Spent—number of hours per week or month and total hours
  • Responsibilities and/or Accomplishments associated with each activity (15 words)

PDF (or .pdf – portable document format) – Use this form to fill in your activities or essay answers and then print the form (you can’t adjust the columns).  To use this, you will need Adobe Acrobat Reader on your computer.  If you do not have it, download it for free by clicking on the link below and following the instructions.

Word Doc (or .doc – Microsoft Word document)– Use this if you can open Microsoft Word documents and you want to either save the document on your computer or would like a little more flexibility (e.g., want more space for your Volunteer activities and less space for your Work activities).

RTF (or .rtf – rich text format) – Use this if you cannot open Microsoft Word documents and can’t or prefer not to use PDFs.
