Activities Chart

One page only!

eApp Users: Use the online form available as part of the OSAC eApp. Prior to November 2 when the eApp is available online, you may use the Activities Chart template as a worksheet. You will not be able to import the worksheet into your eApp.

Paper Applicants: You may use the template or create your own.

Your Activities Chart must include each of (and only) the following three categories in the format shown below:

  •   School Activities - Category A
  •   Volunteer Activities (Community/Family) - Category B
  •   Paid Work History - Category C

Sample 2010-11 Activities Chart

General Instructions for the Online eApp and for Paper Applications
  • One page only!
  • You must have at least one activity from each category (A, B, and C). If you do not have any activities for one of the categories, certify that you have no activity for that category (the online eApp inserts an activity with activity title "No activity for category X" where X is category A, B, or C).
  • Each activity must include:
    • The activity category (A, B, or C)
    • A short title for the activity
    • Dates From: month and year of when you first began that activity
    • Dates To: either use 'To present' if you are currently doing this activity, or
      enter month and year when you last did that activityyou participated (from—to)
    • Time Spent—number of hours per week or month and total hours; give your best estimate (for paper applicants:  don’t say "varied")
    • A short description of the responsibilities and/or accomplishments associated with the activity (up to 125 characters)
  • Be brief. Pinpoint activities where you showed leadership, decision-making, and organizational skills, and areas where you received special recognition.
  • Too many activities to fit on one page? Combine the same type of activity over multiple years into one activity.  For example, combine "Junior Varsity Football" for 08/2008 to 11/2008 with 130 total hours and "Varsity Football" for 08/2009 to 11/2009 with total hours 150 into one activity: "JV/Varsity Football" for 08/2008 to 11/2009 with 280 total hours, and description "In fall, practice 4 times/week, play 1 game/week. 1st year JV center; rest varsity linebacker."
  • Be sure that your activities are categorized correctly.  Note that the 2009-10 Activities Chart had different categories (the old activities chart had category A as School but also Family and Community activities, and category B as only Volunteer Service).
  • Meet the deadline!  If you fail to get these materials to OSAC by the March 1, 2010 scholarship deadline, you will be disqualified.
Specific Instructions for the Online eApp, Activities Chart Section

These instructions apply only to those applicants who use the online eApp.

  • There is a limit of 20 activities.
  • To add an activity, click on the 'Add New Activity to List' button.
    1. Select the activity category.
    2. Add a short title for the activity.
    3. Date From: month and year of when you first began that activity.
    4. Date To: either check 'To present' if you are currently doing this activity, or enter month and year when you last did that activity.
    5. Time spent: hours per unit (select the unit from the dropdown list), and total hours.
    6. A short description of the responsibilities and/or accomplishments for that activity (up to 125 characters).
    7. Click on the 'Add this activity' button and the activity will be added to your list, or click on the 'Cancel' button and the activity is not added.
  • To update an activity in the list, click on the 'Edit' link by the activity.  Make the changes and click on the 'Update this activity' button, or click on the 'Cancel Update' button to cancel the update of the activity.
  • To delete an activity in the list, click on the 'Delete' link by the activity.  Confirm the deletion, or click on the 'Cancel Delete' button to cancel the deletion of the activity.
  • For the Oregon scholarship application, to get activity data from your Oregon Career Information Systems (CIS) Career Planning Portfolio, click on the 'Import CIS Activities' button.  After importing, edit your OSAC Activities Chart so you have no more than 20 activities, so you have at least one activity from each category, and so all activities have the required data.  When importing the data, eApp truncates the activity description to 125 characters, so make sure that each description accurately reflects your activity.
  • To go to the eApp Home page, click on the 'eApp Home' icon.
  • To continue to the Personal Statements, click on the 'Continue' icon.
  • To go back to your scholarship list, click on the 'Previous' icon.
  • Submit your Activities Chart by the March 1, 2010 scholarship deadline by submitting your eApp application.  If you have more than 20 activities, or all data for each activity is not supplied, or you do not have at least one activity for each of the 3 categories, you will not be able to submit your eApp.

Specific Instructions for Paper Applications, Activities Chart Page

These instructions apply only to those applicants who choose to submit the OSAC scholarship application using a paper application form, rather than the online eApp.

  • The Activities Chart is one page only! OSAC will discard additional pages.
  • Use white paper; use 9-12 pt. type size and an easily readable type style (e.g., Times New Roman)   OSAC will disqualify your application if the font size for the Activities Chart is too small or unreadable.
  • You can use the template or create your own as long as your Activities Chart is complete.
  • If you have an Oregon Career Information Systems (CIS) Career Planning Portfolio, you can generate a paper OSAC Activities Chart in CIS.
  • Write your legal name and the last 4 digits of your social security number on the top of your Activities Chart.
  • The sort order of the activities in your activities chart should be:
    1. First the activity category (A, then B, then C)
    2. Then the most recent activity (latest "To date")
    3. Then the longest duration activity
    4. Then the activity title
  • For the time spent in the activity, enter the number of hours per week or month, whether the hours are per week or month, and total hours.  Don't say "varied" hours.
  • Submit your Activities Chart by the March 1, 2010 scholarship deadline by mailing or delivering your complete application packet (signed application form and certification, Activities Chart, 4 Personal Statements, and appropriate transcripts).  If you fail to get all application packet materials to OSAC by the deadline, you will be disqualified.

If you wish to be considered for the Early Bird scholarships and an early review of your application, your deadline for submitting your complete application packet is Monday, February 16.
If you need to make changes to your application after you have mailed or hand-delivered it, click here.
If you need to know that OSAC received your scholarship materials, use a delivery service that gives you a return receipt. Because of the volume of materials that OSAC receives, we are unable to provide you with this information.
