Activities Chart

One page only! Use the online form available as part of the OSAC eApp. Or if you prefer, use the template or create your own. Your Activities Chart must include each of (and only) the following three categories in the format shown below:

A. School/Family/Community Activities
B. Volunteer Service
C. Work for Pay

Activity Chart Sample

Under each category, you must list:

Dates you participated (from—to)
Time Spent—number of hours per week or month and total hours: don’t say “varied”—give best estimate
Responsibilities and/or Accomplishments associated with each activity

Be brief; pinpoint activities where you showed leadership, decision-making, and organizational skills, and areas where you received special recognition.

Keep the chart to one page; include your name, last four digits of your Social Security Number (SSN), and your 5-digit eApp number at the top.

Submitting Your Activities Chart

Submit your Activities Chart as part of your eApp by the March 2 scholarship deadline. Your full application packet must include your:

Your eApp online form (includes Activities Chart, required four short essays, uploaded transcripts and other documents, if required)
Your signed application form (Paper applicants, only)
Activities Chart (available online as part of application or as a separate template)
Four Short Essay Answers (available online as part of the application or as a separate template)
Appropriate transcripts
Other required documents (e.g., additional essays, birth certificate, military papers)

Note: Paper applicants, make sure to write your legal name and the last four digits of your social security number on the top of each page of the documents you enclose with your application (Activities Chart, required four Short Essays, and other required documents).

If you fail to get these materials to OSAC by the deadline, you will be disqualified.

Your eApp or paper application and required documents must arrive at OSAC by the appropriate deadline:

Monday, February 16: EARLY BIRD Review

Monday, March 2: FINAL Deadline. No Exceptions

If you wish to be considered for the Early Bird scholarships and an early review of your application, your deadline for submitting your complete application packet is Monday, February 16.

If you need to make changes to your application after you have mailed or hand-delivered it, click here.

If you need to know that OSAC received your scholarship materials, use a delivery service that gives you a return receipt. Because of the volume of materials that OSAC receives, we are unable to provide you with this information.
