Transcripts

A missing or incomplete transcript is the #1 reason for rejected applications for OSAC scholarships. Read these instructions to ensure that you are submitting what OSAC needs to process your application.

Follow these instructions carefully!

  • Transcripts must list your name, your school’s name, each course taken, grades, grade points, and credit hours.
  • OSAC does not require official transcripts. You can submit transcripts printed from your high school or college website, as long as they include your name, your school’s name, each course taken, grades, grade points, and credit hours. If you are awarded a scholarship, you may be required to provide official transcripts.
  • OSAC will NOT accept the following documents as replacements for transcripts: Report Cards, Grade Reports, Degree Audits, Registration Reports, and Transfer Articulations.
  • You are strongly encouraged to have your transcripts scanned as a single file that can be uploaded to the eApp, when prompted online, or transmitted by your school Registrar's office directly to OSAC through the IDTS Network or the OSAC web portal. (See information below)

Decide what type of student you are to determine the type of transcript to submit:

If you are:

  • A Graduating High School Senior as of 2009-10
  • A High School Graduate with No College Coursework (graduated before  2009-10)
  • An Adult Student with No College Coursework (never graduated from high school, no GED, not home-schooled)
     
    1. Submit transcripts showing your coursework and grades through the first semester, second quarter, or first trimester, depending on your school’s calendar, of your senior year. These transcripts are usually available in January or February.
  • A Home-Schooled Student from Oregon, are not yet enrolled in college, submit all three of the following:
     
    1. A copy of the Confirmation of Enrollment letter on file at your local Educational Service District (ESD).
       
    2. A copy of the results of a tenth-grade standardized achievement test, required for all home-school students who have registered with their ESD.
       
    3. A transcript from your home-school teacher describing your coursework and letter grades assigned.
  • A Home-Schooled Student from outside Oregon, submit the following:
     
    1. Documentation of what your home-school state requires for a transcript from a home school.
       
    2. A copy of all the documents that are required by your home-school state.
  • GED Student
     
    1. If you received a General Education Development (GED) certificate and have never attended college, submit your Transcript of GED record.
       
    2. If you have not completed your GED requirements by the time you submit your application, submit your most recent score report and a letter explaining when you plan to complete your requirements.
  • College Students, Freshman (completed one semester or term of college coursework at the time of application)
  • College Students, Continuing or Returning (will not have completed a bachelor’s degree prior to 2010-11)
  • College Graduate Student
     
    1. Submit transcripts of all college coursework taken between August 1999 and fall quarter/semester 2009. For college freshmen (as defined above), a high school transcript is optional, but recommended as part of the selection process.
       
    2. You may also submit pre-1999 transcripts that document academic achievement, whether from a college or your high school.
       
    3. If you attended more than one college, submit a transcript for each college, including work on prior degrees. You may submit one transcript with transfer credits for other schools, provided it includes grades for all schools and courses; transfer credits alone are not enough.
       
    4. If all your college coursework is completed before 1999, you must submit either those college transcripts or your high school transcripts.

Instructions for paper applicants
When mailing or hand delivering your application to OSAC by the deadline, make sure to include at the top of each page of all documents submitted your name and the last four digits of your Social Security Number (SSN).
